Abstract
Background : Targeted agents, such as sorafenib and regorafenib, are proven to be beneficial in treating metastatic hepatocellular carcinoma (HCC), but their effects are limited due to the severe toxicity and life gain of only 3 months.

Case presentation : Here, we present the case of a 65-year-old male, hepatitis B virus carrier with a metastatic HCC in Dec. 2015. Sorafenib, the standard first-line chemotherapy, was administered, but he could not take it anymore 1 month later due to the adverse effects including skin rash and hand foot syndrome of grade III based on the Common Terminology Criteria for Adverse Events (CTCAE), version 4.0. Alternatively, he received Viscum album extract subcutaneously starting in Jan. 2016. After dose adaptation to 20 §· twice a week, the treatment is still ongoing to the present (Feb, 2018). With an initial partial response of the metastatic lesions, he experienced prolonged survival, compared to the average survival times, and little toxicity.

Conclusion : We suggest that subcutaneous Viscum album extract could be a candidate for metastatic HCC patients who cannot tolerate conventional treatment.
